Reemployment Assistance Appeals Commission Request For Review English Español Kreyol IMPORTANT PLEASE READ THIS ENTIRE NOTICE The appeal form following this page is only for submitting an appeal of an adverse decision of an appeals referee to the Reemployment Assistance Appeals Commission(Commission). To use this form, you will be required to include a valid docket number of the appeals referee decision. To submit your online appeal to the Commission you will need to acknowledge that you have read this notice and click the NEXT button at the bottom of this page, then complete the online Request for Review Form and click the Submit Form button. If you use this form for any purpose other then to file an appeal of an appeals referee’s decision, the commission will not be able to respond to your submission or take any action. The Commission understands you may have questions about your claim for benefits, the status of your claim, or a problem with your claim, but the commission cannot answer these questions for you. Please contact the Florida Department of Commerce (Department) at (800) 204-2418 to obtain the assistance you need. If you want to submit an appeal of an adverse determination notice to the Department’s appeals referees, you will need to file your appeal with the Department’s Office of Appeals. You may do so via the Department’s CONNECT system, or online via the Department’s RA Help Center .
